1. In consideration for receiving permission to participate in the ________________/Realjitsu Academy Seminar on this day of _____/_____/__________, I hereby RELEASE, WAIVE, DISCHARGE, and COVENANT NOT TO SUE the Realjitsu Academy (also known as The Realjitsu Eclectic Functional Fighting Methods Development Academy), their officers, owners, directors, servants, agents, employees, volunteers, members, instructors, coaches, trainers, participants, students, sponsors, promoters, advertisers, affiliates, subsidiary's or lessors of the premises on which the event is to take place (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and cause of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any property belonging to me, whether CAUSED BY THE NEGLIGENCE OF THE RELEASEES, other participants, myself or otherwise while participants in such activities, or while in, on or upon the premises where the activity is being conducted.
2. To the best of my knowledge, I am fully able to participate in the activities. I am fully aware of the risk and hazards connected with activity including but not limited to the risks as noted herein, and I hereby elect to voluntarily participate in said activities knowing that the activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILTY FOR ANY RISK OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH AND PARALYSIS, that may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in such an activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES, other participants, myself the instructor or otherwise.
3. Further I hereby AGREE TO INDEMNIFY AND HOLD HARMLESS the RELEASEES from any and all loss, liability, damages or costs, including court costs and attorney's fees, that may incur due to my participation in said activity, WHETHER CAUSESD BY THE NEGLIGENCE OF RELEASEES, other participants, myself or otherwise.
4. It is my express intent that this Release and Hold Harmless Agreement shall bind both me and the members of my family and spouse (if any), if I am alive, and my heirs, successors, executors, assignees, personal representatives and legal representatives, if I am not alive, and shall be deemed as a RELEASE, WAIVER, DISCHARGE, AND COVENANT NOT TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Wisconsin.
5. I UNDERSTAND THAT THE REALJITSU ACADEMY WILL NOT BE RESPONSIBLE FOR ANY MEDICAL COSTS ASSOCIATED WITH ANY INJURY I MAY SUSTAIN.
6. I further agree to become familiar with the rules and regulations of the Realjitsu Academy concerning participant conduct and not to violate said rules or any directive or instruction made by the person or persons in charge of said program and that I will further assume the complete risk of any activity done in violation of any rule or directive or instruction. I also understand these rules may be violated by others which may result in me being injured or my property being damaged.
7. I also understand that I should, and am urged by the Realjitsu Academy to obtain adequate health, accident and life insurance to cover any personal injury to myself that may be sustained during the program or the transportation to and from said program.
8. I realize that in participating, I may be using weapons, bladed, spiked, blunt, flexible, projectile, real or replica.
9. I understand that The The Realjitsu Academy is not responsible for my use or misuse of any information, knowledge or technique that I may learn while attending a Realjitsu Academy Event.
10. I understand that I am attending this event at my own risk and I have the right to refuse at any time to participate in any part of the event if I believe it to be beyond my physical, mental or emotional ability or for any other or unstated reason.
11. I understand that it is my responsibility to seek a physicians approval of my physical, mental and emotional state and ability to attend said event before attending and that The Realjitsu Academy urges me to do so.
12. I give a complete and unconditional release of any and all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full legal force and effect.
IN SIGNING THIS RELEASE AS A PARTICIPANT IN The ________________/RealJitsu Seminar, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understanding it and signing it voluntarily as my own free act and deed; oral representation, statements or inducement, apart from the forgoing written agreement, have been made; I am at least eighteen (18) years of age and fully competent; or, if I am under eighteen (18) years of age, my parent or legal guardian's approval and signature have been obtained; and I execute this Release for full, adequate, and complete consideration fully intending to be bound by the same.

I have read the forgoing Waiver of Liability and Hold Harmless Agreement, understanding it and sign it voluntarily as my own free act and deed; no oral representations, statements inducements, apart from the foregoing written agreement, have been made; and I execute this Release for full, adequate, and complete consideration fully intending to be bound by the same.
